3、我對(duì)在連續(xù)而激烈的比賽中可能發(fā)生的危險(xiǎn)已有充分的認(rèn)識(shí)，我保證本人的身體健康狀況能夠承受本次比賽，并具備近期健康檢查證明書(shū)。

4、我保證：本人在比賽中因受傷、遭遇意外事故或發(fā)生疾病時(shí)，對(duì)受到的急救處理方法及結(jié)果不提出異議。

5、我本人、我的家屬、親戚和保護(hù)人已充分理解本次比賽的內(nèi)容和意義，并認(rèn)可我本人參賽。在比賽之前我已作了人身保險(xiǎn)。因此，我保證：我自己、親屬及遺囑執(zhí)行人、財(cái)產(chǎn)繼承人，對(duì)本人在比賽中或參加由本次比賽委會(huì)組織的與比賽有關(guān)的活動(dòng)時(shí)，不論什么原因所致的傷、病、亡或出現(xiàn)的后遺癥等，不對(duì)比賽組委會(huì)提出任何起訴。所造成的損失由保險(xiǎn)公司和本人承擔(dān)，比賽組委會(huì)不再提供因此而造成的損失費(fèi)。

6、我保證：允許比賽組織者自由使用和制作與比賽有關(guān)的我的名字、照片等。并承認(rèn)與本次比賽有關(guān)的新聞、廣播、電視等商業(yè)宣傳報(bào)道。

7、我保證：由于自然災(zāi)害等客觀原因造成比賽取消或變更比賽時(shí)間、內(nèi)容所引起的有關(guān)費(fèi)用問(wèn)題，服從比賽組委會(huì)的決定；由于我個(gè)人的情況而不能參加比賽的，不要求退還參賽時(shí)所交納的各種費(fèi)用。

8、我確認(rèn)：對(duì)參賽所進(jìn)行的上述保證已充分理解，并承認(rèn)本保證書(shū)解釋權(quán)屬比賽組委會(huì)。本人自愿簽字。

十一、本人保證在簽署本保證書(shū)時(shí)，本人并未委托任何個(gè)人或公司擔(dān)任本人在文化、娛樂(lè)、演藝等相關(guān)領(lǐng)域之經(jīng)紀(jì)人；本人并承諾自簽署本保證書(shū)之日起至賽事結(jié)束期間，本人亦不會(huì)與任何個(gè)人或公司在上述領(lǐng)域建立委托經(jīng)紀(jì)關(guān)系。

十二、本人保證簽署本保證書(shū)及履行本人在本保證書(shū)項(xiàng)下所作之承諾將不會(huì)造成違反或構(gòu)成不履行以本人為一方的或?qū)Ρ救擞屑s束力的任何合同；或違反任何相關(guān)法律或法規(guī)、或任何政府機(jī)構(gòu)的任何命令、判決或法令。本人保證自簽署本保證書(shū)之日起，本人將不會(huì)與任何個(gè)人、公司或機(jī)構(gòu)簽署與本保證書(shū)相沖突之文件或協(xié)議。

十三、本人保證在賽事進(jìn)行中，無(wú)論本人因任何原因中途退出賽事，本人均按照賽事組委會(huì)之要求公布退出原因。

十四、本人同意若有違反本保證書(shū)項(xiàng)下任何承諾，本人愿意承擔(dān)因此所產(chǎn)生之全部法律責(zé)任，包括但不限于賠償賽事組委會(huì)之全部損失（包括直接損失和間接損失），且賽事組委會(huì)有權(quán)取消本人《超級(jí)男女》稱(chēng)號(hào)及參賽資格。

十五、若本保證書(shū)中的某一條款或某一條款中的部分表述無(wú)效，仍不影響其他條款或其他描述的法律效力。

十六、本人保證在簽署本保證書(shū)時(shí)已經(jīng)詳細(xì)閱讀并理解了本保證書(shū)的全部?jī)?nèi)容，本人同意無(wú)條件遵守此保證書(shū)，且在完全自愿情況下簽訂本保證書(shū)。
